In accordance with article 13 of Legislative Decree 196/03 stipulating the “Code regarding the protection of personal data” RECORDATI S.p.A. (“RECORDATI”) hereby informs you about its processing of the personal data, provided by yourself, to be done in full compliance with applicable laws and regulations and only for the purposes described below.

1. Object of the processing

  a) The personal data (your name, address, telephone number, e-mail address) and such further, sensitive data (indicating health status) as you will give us via e-mail or telephone.

2. Purposes and methods of processing

  a) The processing of information concerning yourself shall be governed by principles of rectitude, legality and transparency, and of the protection of your privacy and rights.
  b) The processing will be carried out both by means of computer and on paper; your data are subject to collection and storage both in computer databases and in paper files.

3. Identity of the Controller of data processing

  a) The Controller of the processing of your data, as per law, is RECORDATI for reports of adverse reactions relative to products of Recordati, with headquarters at Via Matteo Civitali n° 1, 20148 Milan, Italy.

4. The Controller has appointed the heads and subordinates in charge of the processing of personal data at the company Departments and Divisions involved therein. Their names shall be made known as appropriate, in relation to the needs of a specific case, upon request addressed to RECORDATI.

5. Rights of the provider of data

  a) Article 7 of Legislative Decree no. 196/03 (the text of which is reproduced hereinafter) recognises certain of your rights, amongst which the right to be fully informed as to which of your data are in our possession, the purposes for which they have been collected and the manner in which they are processed, as well as the right to cause the data to be updated or deleted.
  b) You may exercise your rights as recognised by law by written communication addressed to RECORDATI, with headquarters at Via Matteo Civitali n° 1, 20148 Milan, Italy.

The following is the text of art. 7 of Legis. Decree 196/03 - “Right of access to personal data and other rights of the provider thereof”-

1. The provider of personal data shall have the right to obtain confirmation as to whether personal data concerning him/her exist irrespective of the recording thereof, and to obtain communication of such data in intelligible form.

2. The provider of personal data shall have the right to be informed:

  a) of the source of such data;
  b) of the purposes and methods of its processing;
  c) of the logic applied to the processing, if carried out with electronic means;
  d) of the identity details of the data controller, the persons processing data and the representative designated as per art. 5(2);
  e) of the entities or categories of entity to which the personal data may be conveyed and which may obtain knowledge of said data in their capacity as designated representatives in the national territory, as processors of data or as persons in charge of processing.

3. The provider of personal data shall have the right to obtain:

  a) updating, rectification or, when having an interest therein, completion of the data;
  b) deletion, anonymisation or blocking of data that have been processed unlawfully, including data whereof the retention is unnecessary for the purposes for which they have been collected or subsequently processed;
  c) certification of the fact that the entities to which personal data have been conveyed or disseminated have been notified of the operations as per points “a” and “b” of the present clause, including as regards the content of said data, unless discharge of the present requirement should prove impossible or entail an effort manifestly disproportionate to the right protected.

4. The provider of personal data shall have the right to object, in whole or in part:

  a) on legitimate grounds, to the processing of personal data concerning himself/herself, even should such data be relevant to the purpose of the collection;
  b) to the processing of personal data concerning himself/herself when carried out for the purpose of sending of advertisements or for direct sales or for the effecting of market research or commercial communication.
Statement of acceptance of the foregoing terms, and of consent to the processing of personal data
The undersigned, having examined the foregoing notice,
hereby consents  
to all operations of processing of his/her personal data for the purposes and in the manner stated in section 2 of said notice.
All of the said activities shall be carried out by RECORDATI in accordance with laws and regulations in force.
